Medicaid Waiver Application Guide

Step 1

Review Eligibility Requirements

Start by thoroughly reviewing the Medicaid eligibility guidelines, particularly the spend-down process.

Step 2

Initiate Contact with the Aging Disability Resource Center (ADRC)

The applicant or Power of Attorney (POA) should contact the ADRC for a telephone interview to discuss the client's needs.

This can be done through the following phone numbers:

  • Local: 505-476-4937

  • Toll-Free: 1-800-432-2080

During the call, request an Expedited Freedom of Choice Application, which is necessary for the Medicaid Waiver process.

Step 3

Complete The Application

The applicant or POA should complete the Freedom of Choice application carefully and thoroughly to ensure all required information is included.

Step 4

Prepare Supporting Documents

Gather and attach the following documents to support the application:

  • Last three months of bank statements.

  • Copies of Medicare ID card, any additional insurance ID cards, and New Mexico ID.

  • Copy of the Power of Attorney documentation.

  • Invoice from assisted living or nursing home, if applicable.

  • Recent tax form showing Social Security benefits.

  • Retirement pension statement or relevant tax form.

  • Doctor's letter detailing the applicant's daily living and/or cognitive abilities.

  • Documentation from the assisted living or nursing home outliving the daily living assistance being provided.

  • Receipts documenting the spend down assets.

Step 5

Follow-up with ADRC

The applicant or POA should periodically contact the ADRC to check the status of the application.

Step 6

Transition to Medicare/Medicaid Plan

Once the Medicaid waiver is approved, the applicant or POA can transition their medicare plans to a Medicare/Medicaid plan suitable for their new status.
